Baptism Requirments
Key Documents Required
- Copy of Child's Birth Certificate
- Marriage Certificat (If applicable)
- Sacramental Documentations: Copy of Baptisimal, First Communion, and Confirmation certificates. All three required from the Godparents.
- Godparent Affadavit of Eligbility Certificates (required of both Godparents from the Godparent's home parish signed by their parish priest)
- Baptisimal preperation class certificate (required of both the parents and Godparents.
Requirment for parents
- Intent: Must have a "reasonable hope" or promise to raise the child in the Catholic faith.
- Resgistration: Usually required to be registered and active members of the parish.
- Preperation: Must attend a Baptisimal Class.
- Documentation: Parish must recieve a copy of Child's birth certificate and Baptismal Class Certificate of completion.
Requirments for Godparents (Sponcers)
- Number: At least one Godparent, with a maximum of two (one male, and one female).
- Age: Must be at least 16 years old.
- Initiation: Must not be the biological parents. Must have recieved all three sacrments of initiation: Baptisim, First Holy Communion, Confirmation.
- Practice: Must be actively living their faith, attending Mass, and, if married, married within the Catholic Church. Not married civilly outside the church, or cohabitating.
- Sacremental Documentation: Parish office must recieve a copy of the Baptisimal, First Holy Communion, amd Confirmation certificates of both Godparents.
- Other Documentation: Parish Office needs to recieve a copy of Baptisimal Class Certificate of completion and Affidavit of Eligibility Certificate.
